Description
Issue card for flying clothing for J. B. Chrisp (NZD 2607) TyMidM(A) RNZMVR. Sets out the reference numbers, items and quantities received and signed for by J. B. Chrisp. In the returns column is a pencil note: R.A.F. Thornet Island and 836. On the back one line for Tubes, Speaking, earpieces is filled in, signed by J. B. Chrisp and dated 2 Oct 1941. The form is stamped with a date of issue. It reads Clothing Officer H.M.S. "St. Vincent" 2 Oct 1941. The signature is W ? Wheeler with Paymaster Lieutenant stamped underneath the signature.
