Front: Paper sheet with printed statement concerning presentation of medals from Minister of Defence, on behalf of Prime Minister and New Zealand Government.
Back: list of all medals presented in this fashion- 1939-45 Star, Atlantic Star, Air Crew Europe Star, Africa Star, Pacific Star, Burma Star, Italy Star, France and Germany Star, Defence Medal, War Medal 1939-45. Bottom torn and info missing: 'War Medal 1939 - 1945' was to be described. Possibly one further medal listed which is unable to be seen due to tearing.

Front: 'The Minister of Defence has pleasure in presenting the enclosed Awards with the compliments of the Prime Minister and New Zealand Government, and in sincere appreciation of loyal service rendered during the war of 1939-45.' Printed in black ink with the New Zealand coat of arms printed in red.
