Description
The National Health Insurance Emergency card is a double-sided sheet of paper with black print on both sides. The front side has a crest on top at the centre. Flanking the crest are the words: Employed Contributor Y, and England. It has instructions for using the card, a personal information section and a Society or Insurance Committee section. There are six orange and white National Health Insurance 10d stamps attached to the card, dated from 5 Nov - 10 Dec 1920. There are black ink blotches on the card and C.K.Mills is handwritten in the top right corner. The reverse of the card has information on: Assignment or transfer, Production of card + lost card. At the bottom is printed: [M574] 4195/367 12000m 5/12 R-6 G&S.
